Blooming Leaf Wellness LLC provides functional medicine education, health coaching, nutritional guidance, and wellness consultation. Our services are educational and supportive in nature. They are not the practice of medicine, nursing, psychology, or any other regulated clinical profession. We do not diagnose, treat, cure, or prevent any disease.
Use of this Site, completion of a self-assessment, or general communications does not create a doctor-patient, therapist-client, or fiduciary relationship. A professional relationship arises only upon execution of a written client services agreement.
Information and services provided by Blooming Leaf Wellness are not a substitute for evaluation, diagnosis, or treatment by a licensed physician or other qualified healthcare provider. Always consult your physician before starting, stopping, or changing any medication, supplement, diet, exercise program, or other health-related activity. If you are experiencing a medical emergency, call 911 or your local emergency number.
Our services are available to adults 18 and older. You represent that you are of legal age and have authority to enter into these Terms.
Services are delivered via telehealth from the State of Illinois. By engaging our services, you acknowledge that consultations are educational and not state-licensed clinical treatment. We may decline to work with clients in jurisdictions where doing so would be inappropriate for our scope of practice.
Specific payment, scheduling, cancellation, and refund terms are set forth in the individual client services agreement signed at the start of an engagement. Initial discovery calls are complimentary and not refundable in cash.
Clinical communications between practitioner and client occur exclusively through our secure, HIPAA-compliant client portal. Casual emails or public website forms are not appropriate channels for sharing sensitive health information.
All content on this Site — including text, images, logos, and the “Blooming Leaf Wellness” name and marks — is owned by Blooming Leaf Wellness LLC and may not be reproduced, redistributed, or commercially used without prior written consent.
